Of Derby, NY Entered into rest December 7, 2015 in the Center for Hospice and Palliative Care, due to complications of Neurofibromatosis he 13. Loving son of Nicholas and Monique (Hebert)Bublyk; brother of Drew Bublyk; grandson of Alain C. Hebert, Elizabeth M. Cyr, Victor and Barbara Bublyk; great grandson of Jenny Weinar; nephew of Sandra and Patrick Hebert and Jennifer Caverly. Evan was a member of Carly's Club at Roswell Park, Camp Good Days and Brainy Camps/Camp New Friends. Family will be present to receive friends Saturday from 1-3 and 6-8 PM at the Addison Funeral Home Inc. 262 N. Main St. Angola, NY. Memorials may be made to the Evan Bublyk Memorial Fund, PO Box 705, Derby, NY 14047
