Passed away, Saturday June 18, 2016, in his Angola, NY home, he was 67. Loving father of Joseph (Kathy) Augustyniak, Frank Augustyniak and the late Edward. Brother of Patricia ( Patrick) Zemko, David(Peggy), Joseph(Kim) Augustyniak the late Joyce Dynarski, Sharon and Bertha Augustyniak. Loving companion of Patricia Small. Grandfather of Joseph and Gracie Lee Augustyniak. Also survived by several grandchildren and many nieces and nephews. Services are private. Arrangements by the ADDISON FUNERAL HOME, INC. 262 N. Main St. Angola, NY
