Daniel J. Polak Sr., of Angola, NY died Thursday February 5, 2015 in Lake Shore Hospital, he was 84. Loving husband of Lucille Polak; father of Daniel (Hollis) Masters, Diana (Raymond) Cunningham, Debra (Thomas) Azzarella, Darlene Polak, David Polak and the late Mildred Polak; loving uncle of Jean Cirino, Samuel Polak and the late Roseanne Griffin; brother of Josephine Bueme, Lucille Polak and the Eugene Polak, Richard Polak and Dorothy Walnilci. Also survived by many grand and great grandchildren, nieces, nephews and great nieces and nephews. Friends may call Tuesday from 2-4PM at the ADDISON FUNERAL HOME, INC. 262 N.
