Jerry Skudlarek, of Derby, NY died Wednesday June 4, 2008, in Lake Shore Hospital, he was 58.� Loving husband of Donna Skudlarek and the late Louise Perrin.� Father of Nick Skudlarek.� Step father of Danielle (Christopher) Wagner and Dayna ( Jason) Murphy.� on of Helen (Late Andrew) Skudlarek;� Brother of Susan, Ronald, Michael and the late Joseph.� Also survived�by 4 grandchildren; many nieces and nephews.� Friends may call Friday from 7-9Pm and Saturday 1-5PM at the ADDISON FUNERAL HOME where a prayer service will be held following Saturdays visitation.���
